2011 Christmas Jar: This year your FOA Gift Basket purchase will be extra special and local. A second grader at one of our daughters’ schools (his name is Joey) is in need of a bone marrow transplant. Our Second Annual Christmas Jar Fund will be on Joey’s behalf, with $5 to $10 from the sale of each gift basket given to his family on Christmas Day.

IMPORTANT: If you are purchasing multiple gift baskets, you must use one Holiday Gift Baskets Form for each DELIVERY LOCATION. If you are purchasing multiple baskets all going to the same place (such as a business), one form will do – just note in the Special Instructions section the names of the individuals receiving the baskets.
